The levels of MMP-2, TIMP-2 and its ratio in blood serum were studied in order to identify predictors of severe AKI for initiating early renal replacement therapy after CABG. The study engaged 60 patients, from 45 to 74 years old, with CHD before and after CABG without diabetes mellitus. Statistically signifi cant differences of MMP-2, TIMP-2 and its ratio in the blood serum were revealed. Determining the ratio of two structural kidney damage markers (MMP-2, TIMP-2) in patients on the 2nd day after CABG allows to detect AKI. The dynamics of the MMP-2 / TIMP-2 ratio in the two compared groups with AKI was different. In the 1st group, the value of ratio signifi cantly decreased on the first day after CABG, significantly increased on the 2nd and reached high rates on the 7th day. In the 2nd group, there was a wave dynamics of MMP-2 / TIMP-2 ratio: increase on the first day, a signifi cant decrease on the 2nd and a twofold increase on the 7th day. Diagnostic and prognostic significance of the MMP-2 level was found in patients with CKD at all steps of monitoring.
Изучен уровень MMP-2, TIMP-2 и их соотношения в сыворотке крови с целью выявления предикторов тяжелого течения ОПП для начала проведения ранней заместительной почечной терапии после АКШ. В исследовании приняли участие 60 пациентов с ИБС обоего пола от 45 до 74 лет до и после АКШ. Выявлены статистически значимые различия MMP-2, TIMP-2 и их соотношения в сыворотке крови у пациентов с ОПП до и после АКШ разных групп. Динамика индекса соотношения MMP-2/TIMP-2 в двух сравниваемых группах больных с ОПП была разной. У пациентов I группы в первые сутки после АКШ показатель достоверно снижался, с последующим существенным нарастанием на 2-е и высокими значениями к 7-м суткам после АКШ. У пациентов II группы определена волнообразная динамика MMP-2/TIMP-2: повышение в первые сутки, значимое снижение на 2-е и двукратное увеличение на 7-е сутки. У пациентов с ХБП установлена диагностическая и прогностическая значимость уровня MMP-2 на всех этапах мониторинга.
